ACKNOWLEDGMENT OF RISK

I, the undersigned, understand that wrestling and related physical activities are inherently demanding and potentially dangerous. Participation involves risks, including but not limited to: sprains, fractures, concussions, strains, or other physical injuries, as well as the potential for emotional or mental stress.

I acknowledge and accept these risks, along with all risks arising from participation in wrestling classes, demonstrations, or events offered by Kelowna Wrestling Club, whether at the Club's facilities or external venues. I affirm that the sole responsibility for my personal safety lies with me, including assessing my physical and emotional readiness to participate.

ASSUMPTION OF RESPONSIBILITY

I agree to:

1. Follow all instructions, safety rules, and guidelines provided by the Kelowna Wrestling Club and its instructors.
2. Immediately notify instructors of any discomfort, pain, injury, or unsafe conditions observed during activities.
3. Remove myself from participation if I feel unfit or unsafe to continue.

MEDICAL CONDITIONS AND EMERGENCIES

I certify that I am in good physical condition and fit to participate in wrestling activities. I agree to seek medical advice if I have concerns about my health or fitness before participating. In case of an emergency, I consent to receiving medical treatment, including emergency first aid, and authorize Kelowna Wrestling Club to arrange such care on my behalf if I am unable to do so.

RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to participate in the activities of Kelowna Wrestling Club, I hereby waive, release, and discharge the Club, its owners, employees, instructors, agents, and affiliates ("Released Parties") from any and all claims, demands, losses, damages, or liabilities, including but not limited to those arising from the Released Parties' negligence or equipment failure.

This waiver applies to injuries, illnesses, death, or damage to personal property occurring during my participation in activities or use of facilities and equipment provided by the Club.

I further agree to indemnify and hold harmless the Released Parties from any claims or actions arising out of my actions, omissions, or negligence during participation.

PHOTO/VIDEO CONSENT

I acknowledge that photographs, videos, and other recordings may be taken during participation in Club activities for promotional, educational, or operational purposes. I consent to the use of my image, likeness, or voice in such media without compensation unless otherwise stated in writing.

ACKNOWLEDGMENT AND AGREEMENT

By signing below, I affirm that I have read, understood, and voluntarily agree to the terms of this liability waiver. I acknowledge this document as a binding contract and agree to its terms freely and without coercion.
